Validator contact/airdrop form

Note that the form is no longer used for admissions: right now you need to fill it only if you are eligible for an airdrop.

Simply navigate to the form and start filling out answers in the relevant fields. If you are eligible for an airdrop, the most important one will be the mainnet address where you'd like the funds sent. Please make sure you've pasted the address correctly, as otherwise the funds will be lost.
